1 - Your most recent prescription glasses or written prescription, most recent back-up and any prescription sunglasses.
2 - Your insurance card, SS# and ID.
3 - Your most recent contact lens prescription, this could be written out or just the old boxes.
4 - Your completed office forms printed out from the links below then filled out. If you're a patient that needs more time to fill out forms or just want to save
time in the office the following forms are what you need.
